If your employer offers a health insurance plan but you need help to cover the premium and other costs, Seedco can help you through the Family Health Plus Premium Assistance Program.
The Premium Assistance Program will:
Seedco can help workers whose income is too high to qualify for Medicaid enroll in Family Health Plus, a free health insurance program with the health plan of your choice.
To qualify you must:
- › Be a resident of New York between the ages of 19 and 64
- › Be a U.S. Citizen or fall under any immigration category
- › Have income and resources that fall below the maximum levels allotted for your family size
Premium Assistance Program
If your employer offers a health insurance plan but you need help to cover the premium and other costs, Seedco can help you through the Family Health Plus Premium Assistance Program.
The Premium Assistance Program will:
- › Pay for the your share of the premium (including dependents)
- › Reimburse costs of co-payments, deductibles and co-insurance
- › Cover additional medical benefits
- › Meet the Family Health Plus eligibility requirements listed above.
- › Be enrolled in your company’s health plan
