Strong Fathers, Stronger Families (SFSF) is a program funded through the U.S. Department of Health and Human Services’s Administration for Children and Families. The program is designed to support low-income fathers in New York City enter and success in the workforce while developing closer relationships with their children, partners, and co-parents. Seedco, a national nonprofit organization that advances economic opportunity for people, businesses and communities in need, has been operating the SFSF since 2011. Seedco partners with local community-based organizations to deliver the critical services offered by SFSF.

The case manager will be employed by Seedco’s subcontractor, BronxWorks. BronxWorks helps individuals and families improve their economic and social well-being. From toddlers to seniors, we feed, shelter, teach, and support our neighbors to build a stronger community. The SFSF Case Manager will be responsible for administering direct service interventions for program participants. The Case Manager will co-facilitate daily workshops covering topics related to parenting, relationships and self-empowerment. Intensive case management services will be delivered by the Case Manager via a one-on-one, trauma-informed, strength-based service delivery model.

The Case Manager will be responsible for the identification of employment challenges and subsequent training and resource needs; general career counseling; tracking of participant data; and referrals to supportive services and counseling.

Primary Job Responsibilities Related to:

Direct Case Management and Facilitation

  • Conduct in-depth intake and ongoing assessments.
  • Co-facilitate workshops and lead discussion groups.
  • Develop individual service plans to address immediate challenges and long-term goals; including referrals to community resources.
  • Assess and record participant progress with detailed case noting within 24 hours of interaction.
  • Participate in or lead bi-weekly case conferences


  • Prepare participants for placement into employment opportunities that are in line with their experience, skills and interests.
  • Work closely with Employment Specialists to determine the needs and abilities of each participant, and whether they are ready to begin employment or training.
  • Assist in retention efforts, keeping in regular contact with members once employed to ensure maximum retention milestones are achieved and opportunities for career advancement are explored and secured.

Outreach and Administrative

  • Administer contract-required surveys and submit as needed to Seedco Data Specialist.
  • File documentation and applications; maintain updated contact information in Salesforce
  • Participate in marketing outreach efforts in collaboration with the Program Coordinator and Seedco Program Management
  • Assist in the organization and implementation of special events.

Supervisory Reports: Weekly Case Management Report

Interacts with: Program Coordinator, Employment Specialist, and Data Specialist;


  • Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up.
  • Manage a case load for a diverse client population.
  • Monitor and document client progress toward service plan goals.
  • Document all interactions with, or on behalf of, clients.
  • Maintain familiarity with program resources available on- and off-site.
  • Provide group counseling as assigned by the program director.
  • Coordinate client services with other social service, mental health, medical, employment, educational, child care and other providers.
  • Conduct regular case conferences with clients and service providers as required.
  • Complete program reports on a monthly basis, or more frequently as required.
  • Report to, and meet with, supervisor on a monthly basis.
  • Conduct home visits and other field work, as required.
  • Perform additional duties as assigned.


  • Bachelors degree in social work or another related field of study.
  • One-year of case management or relevant work experience preferred.
  • Engaging personality and detailed orientated skills are essential.
  • Strong oral and written communication, time management and organizational skills are necessary.
  • Proficiency in Microsoft Office suite and other standard business technology is required.


  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand, walk, or sit for long periods of time
  • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork
  • Ability to bend and retrieve objects and/or documents
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
  • Advanced-level professional competency in written and spoken English language is required. Authorization to work in the United States for any employer is mandatory.
